We want your visits to our office to be as pleasant and smooth as possible. The following information will help the process.
When coming to our office for the first time, please bring a form of ID, your current insurance card(s), your health information and current list of medications. You may fill out much of the medical information ahead of time by either downloading the forms available here, or by following the link to NextWeb to fill it out online.
You are expected to pay any co-pays and/or deductibles at the time of the visit. If you give us your insurance information at the time the appointment is made, we will have the opportunity to check for out-of-pocket costs and get some pre-certifications done before you come in. You are responsible to get any referrals from primary care doctors if required to see Dr. Davis. We can help if with this process if needed, but it is better to know in advance. If we do not receive your insurance information, you will be responsible for all visit costs.
We perform procedures in both the Colleyville and Ft. Worth offices and in various outpatient surgery centers. If you desire sedation for a procedure, then it must be done at the surgery center. A 10% Deposit that is non-refundable will be required to schedule surgery due to high demand.
If you are scheduled for surgery either in the office or at the outpatient surgery center, it is important to withhold any blood thinners for 1 week prior to the surgery. Common blood thinners are aspirin, ibuprofen, Alleve, Advil and Excedrin, Plavix and Coumadin. However, you should check with your primary care doctor or cardiolgist to be sure it is okay to stop blood thinners for this long, particularly if you take Plavix or Coumadin. Tylenol does not thin the blood, so you may take it as needed. Please call our office if you have any questions.
We recommend a driver to take you home after surgery, unless it is very minor. If you are having lid surgery on both eyes or having sedation, it is mandatory.
Please do not wear any eye make up if you are scheduled for an eyelid procedure.
Our office accepts most of the major insurances including Medicare, Blue Cross/Blue Shield, United Healthcare, Aetna, PHCS, and Medicaid. We also accept numerous others that are not listed here, so please call our office to check.
We accept most forms of insurance, but please call to confirm if you are not sure. We do not accept Medicaid.
